Why Is This Role Critical?

The Health Services Liaison(HSL) role is responsible for completing and accurately documenting supporting records such as clinical data and assessments including but not limited to post-discharge assessments and facility conformation calls.  The HSL will use defined guidelines to determine when the results of an assessment require escalation to a designated clinician with higher licensure.  The HSL in some cases with approved licensure could be credentialed to complete functionality assessments. The HSL is responsible for inbound and outbound telephone communication, accuracy of database documentation and the completeness of the medical record and naviHealth databases as defined by quality and service standards. This role will be held accountable for productivity, as well as service and quality standards as defined by departmental standards.

What you will be accountable for….

  • Completes and maintains a record of multiple contact attempts to member’s post-discharge.
  • Performs member assessment with scope of licensure
  • Refers to appropriate nH colleague when appropriate, based on defined process and criteria.
  • Documents and reports member and provider complaints to appropriate naviHealth and health plan personnel per organizational compliance policies.
  • Enters documentation in a timely manner and ensures accuracy of all required information into the appropriate naviHealth documentation system.
  • Retrieves patient records from our contracted partners Electronic Medical Record system accurately uploading into the naviHealth documentation system.
  • Inform designated naviHealth personnel when there are discrepancies or omissions in the medical record and/or naviHealth data base.
  • Adheres to organizational, departmental, compliance and regulatory policies and procedures.
  • Manages inbound and outbound telephone communication in a professional manner and according to productivity and quality standards.
  • Answers phone timely and efficiently to enhance customer satisfaction.
  • Attends naviHealth meetings as required.
  • Communicates directly with the member, healthcare providers and/or naviHealth team.
  • Performs all other duties as assigned.
  • Promotes a positive attitude and work environment.

What you will need to be successful……

  • High School diploma or GED required
  • Clinical licensure (EMT/Paramedic, RAD, PTA or COTA) required.
  • Experience in geriatric acute or post-acute care setting strongly preferred.
  • Excellent written and oral communications skills including the ability to communicate with a variety of people and backgrounds including:  health plan members, providers, physicians, and health plan representatives.
  • Detail oriented with ability to prioritize, plan, and handle multiple tasks/demands simultaneously.
  • Proficient with Microsoft Office applications including Word, Excel and Power Point.
  • Microsoft Excel skills.
  • Able to use various office equipment, such as: copier, fax machine, telephone and scanner.
  • Must be proficient in Medical Terminology.

The nice to haves……

  • Bilingual language a plus.
  • Manual dexterity needed for keyboarding and other repetitive tasks
  • Must be able to work in a climate controlled, office environment.
  • Vision and Hearing must be adequate to perform job duties.
  • Ability to read and write in English in order to process paperwork and document information
